Through a Stratification Process, Employees are Identified for Diabetes Interventions
MedWise Diabetes Disease Management System provides the programs employees need to:
- Achieve and maintain optimum blood glucose levels
- Manage co-morbidities related to diabetes, including hypertension with diabetes management
- Prevent the onset of complications by providing innovative diabetes management services including foot exams
- Health and Wellness Program supports maintaining healthy lifestyles and adhering to recommended health management plan
Diabetes Management Program interventions include:
- Comprehensive Employee Health Risk Assessment to identify health risk and assist in creating individualized employee action plans. Plans include both short- and long-term goals to better manage employees diabetes and reduce risks
- Proactive Care Coordinator outbound and responsive inbound calls to help employees achieve treatment goals
- Diabetes Management System records blood sugar test results in HIPAA compliant encrypted information server for 24 hours/7 days a week/365 day a year access
- Management of risk factors and co-morbidities related to diabetes, including hypertension with diabetes management
- Educational opportunities to learn about:
- Chronic conditions, self-management techniques to control and manage the progression of employees diabetes
- Nutritional plan and meal planning preparation guides
- Health coaching and consultation
- Cost-saving opportunities through exclusive use of mail-order for diabetic and ancillary supplies
- Wellness programs that promote healthy lifestyle choices

Note: The progression from Acuity Level 5 to Level 4 and the progression from Level 4 to Level 3 constitute the clinical improvement necessary under the Affordable Care Act’s “health-contingent wellness programs” for employee’s to receive a reward or incentive from their employer.
The A1C, BP, Cholesterol, BMI are acuity level numbers for clinicians use only and not to be construed as targets or goals for employees. This program is voluntary and thus meets standards set forth by HIPAA, ADA, EEOC, GINA, PHI and ADEA directives. This program also includes alternatives for disabled employees to receive rewards/incentives when unable to participate in specified activity.
